All Chieftain Athletes participating in any sport MUST have a current physical on file &
a parent/student consent form signed for Concussion, Head Injury, Sudden Cardiac, Heat Illness.
Effective July 2022, a signed affidavit declaring the student's biological sex at birth is a requirement from student athletes.

Also ALL football players MUST have an insurance policy and verification given to the coach. If any football parent needs accident insurance, the school suggests using Student Insurance Plan from K&K Insurance. Coverage may be purchased at any time during the school year by visiting www.studentInsurance-kk.com. The insurance can also be purchased by any parent, football player or not. It's a great accident policy that covers their student wherever they are!
